General Patient Information
We accept Medicare, Medicaid patients and are members of most health care plans. If you have any questions, please contact the office for contracted provider verification.
The majority of our patients are covered by Medicare. Our first responsibility is to represent the best interest of the patient and to do that best, we have chosen not to be contracted directly with Medicare under their participating requirements. We therefore remain independent to best represent you directly in assuring you receive the best of care. Our first and foremost goal is to see that you receive superlative healthcare.
It is important that you bring your insurance card to every appointment. If you switch insurance companies or change your address, phone number, etc., please contact our office as soon as possible so we can make the necessary changes to your chart and/or account.
You are responsible for the timely payment of your account. If you have co-pay, co-insurance and a non-satisfied deductible, payment is expected at the time of your visit.
Some insurance plans do require a referral for services. It is the responsibility of the patient and their primary care physician to obtain the referral. Please confirm with your insurance company if a referral is needed. If so, contact your primary care physician to verify that a referral has been obtained prior to your appointment.
Method of Payment
All insurance co-payments, co-insurance and non-satisfied deductibles are due at the time of treatment. You may pay by cash, check, and VISA, Discover, MasterCard or debit card. If necessary, we can provide financial counseling. Our billing office can be reached at (210) 616-0008.
